Bulleen is on Manningham Road in Melbourne's east. Sunbury is on Dornoch Drive in Melbourne's north-west. Limited parking is available at both locations and both are wheelchair accessible.
We see patients from Bulleen, Doncaster, Templestowe, Balwyn, Kew and Ivanhoe, and from Sunbury, Diggers Rest and Gisborne. The Skin Clinic operates from Bulleen only.
Referrals
You need a referral from your GP or another specialist before a consultation for surgery. A referral is also needed to claim a Medicare rebate for your specialist consultation. GPs can send referrals by fax to 03 8738 0200, by email to info@evolvesurgery.com.au or via HealthLink (anavekar).
Skin clinic appointments don't need a referral.
